104 SPONTANEOUS ILIOPSOAS TENDON RUPTURE AND PSOAS HAEMATOMA ON ASPIRIN MONOTHERAPY: A CASE REPORT

Abstract Background Spontaneous Iliopsoas Tendon (IPT) rupture is a rare condition, with prevalence of 0.66%, occurring most commonly in athletes. Risk factors for non-athletic IPT include age, female gender and steroid use. Psoas haematomas are associated surgery, anticoagulation coagulopathies. Only handful case reports have psoas haematoma single antiplatelet We present the patient who developed tendon haematoma. Methods A 77-year-old presented four week history declining mobility left hip pain, without trauma. Past medical included chronic lymphoedema an NSTEMI, which she was taking aspirin monotherapy. On examination, had reduced power on flexion tender anterior thigh, normal sensation. CT showed asymmetric thickening iliacus muscle surrounding fat stranding. MRI demonstrated partial tear oedema extending into muscle. Results After several weeks rehabilitation, her deteriorated again. second performed, new expansion stranding right iliopsoas compatible acute haemorrhage. Aspirin held weeks, discharged after completing rehabilitation. Her baseline disimproved required assistance frame discharge. Conclusion atypical causes atraumatic pain older adults. Nevertheless, they can cause significant disability. recommended as gold standard diagnosis. Treatment conservative, focused physiotherapy control to maintain mobility.
